Divorce with Children
- Copy of your Photo ID
- Income: two full months (most recent) of pay stubs; W-2 for the last year, and last year’s tax return
- Insurance: cost of health, dental, vision & life insurance for you and your child/ren.
- Children expenses: cost of daycare, camps, after care, extra-curricular activities and private school; a copy of any child support orders already in place.

Divorce, No Children

Legitimation
To complete forms to file a petition for Legitimation, the father must attend Legitimation for Fathers class before attempting to schedule an appointment at the Family Law Resource Center. Child support MUST be determined in the action unless a child support order is already in place.

Modification
- Copy of your Photo ID
- Bring a full and complete copy of the final Order that you want to modify (ex. final divorce decree with all attachments, child support order, custody order, legitimation order) AND Last 2 most recent paystubs.
- Address of other parent.
Contempt
- Copy of your Photo ID
- Bring in a full and complete copy of the final Order (ex. final divorce decree with all attachments, child support order, custody order, legitimation order).
- Address of other parent.
Adult Name Change
Bring a copy of your birth certificate, driver’s license and social security card.
Answer
- Copy of your Photo ID
- Bring the full and complete packet of papers served to you.
- Your income: 2 full months (most recent) of pay stubs; W-2 for the last year, and last year’s tax return
- Other party’s income: Any paperwork you have showing the other party’s income
- Insurance: cost of health, dental, vision & life for you and your children ( if they are at issue in the lawsuit)
- If Children are involved in the lawsuit: cost of daycare, camps, after care, extra-curricular activities and private school; a copy of any child support orders already in place.
