WHAT DO I BRING TO MY APPOINTMENT?

*Bring your photo I.D.*

Divorce with children:

      1. Income: 2 full months (most recent) of pay stubs; W-2 for the last year, and last year’s tax return
      2. Insurance: cost of health, dental, vision & life insurance for you and your child/ren.
      3. Children expenses: cost of daycare, camps, after care, extra-curricular activities and private school; a copy of any child support orders already in place.
      4. Address of your spouse

Divorce, no children:

  1. Income: 2 full months (most recent) of pay stubs; W-2 for the last year, and last year’s tax return
  2. Insurance: cost of health, dental, vision and life insurance for you
  3. Address of your spouse

 

Legitimation: To complete forms to file a petition for Legitimation, the father must attend Legitimation for Fathers class before attempting to schedule an appointment at the Family Law Resource Center.  Child support MUST be determined in the action unless a child support order is already in place.

  4. Address of the Mother

 

* Modification: Bring three copies of the full and complete final Order that you want to modify (ex. final divorce decree with all attachments, child support order, custody order, legitimation order) or we will charge you $15.00 for us to make the three copies; AND Last 2 most recent paystubs.

* Contempt: Bring three copies of the full and complete final Order that you want to modify (ex. final divorce decree with all attachments, child support order, custody order, legitimation order) or we will charge you $15.00 for us to make the three copies.

* Adult Name Change: Bring a copy of your birth certificate, driver’s license and social security card.

* Answer:

  1. Bring the full and complete packet of papers served to you.
  2. Your income: 2 full months (most recent) of pay stubs; W-2 for the last year, and last year’s tax return
  3. Spouse’s income: Any paperwork you have showing your spouse’s income
  4. Insurance: cost of health, dental, vision & life for you and your children ( if they are at issue in the lawsuit)
  5. If Children are involved in the lawsuit: cost of daycare, camps, after care, extra-curricular activities and private school; a copy of any child support orders already in place.
