WHAT DO I BRING TO MY APPOINTMENT?

*Bring your photo I.D.*

Divorce with children:

      1. Income: 1 full month (most recent) of pay check stubs;
      2. Insurance: cost of health, dental, vision & life insurance for you and your child/ren.
      3. Children expenses: cost of daycare, camps, after care, extra-curricular activities and private school; a copy of any child support orders already in place.
      4. Address of your spouse


Divorce, no children: 2 recent Pay stubs; cost of health, dental, vision and life insurance for you; & Address of your spouse.

 

Legitimation: Child support MUST be determined in the action unless a child support order is already in place.

  1. Income: 1 full month (most recent) of pay stubs
  2. Insurance: cost of health, dental, vision & life insurance for you and your child/ren.
  3. Children expenses: cost of daycare, camps, after care, extra-curricular activities and private school; a copy of any child support orders already in place.
  4. Address of the Mother

 

* Modification: Bring a copy of the full and complete final Order that you want to modify (ex. final divorce decree with all attachments, child support order , custody order, legitimation order); AND Last 2 most recent paystubs.

* Contempt: Bring a copy of the full and complete final Order that you want to modify (ex. final divorce decree with all attachments, child support order , custody order, legitimation order).

* Adult Name Change: Bring a copy of your birth certificate, driver’s license and social security card.

* Answer:

  1. Bring the full and complete packet of papers you were served.
  2. Your income: 1 full month of paystubs (most recent)
  3. Spouse’s income: Any paperwork you have showing your spouse’s income
  4. Insurance: cost of health, dental, vision & life for you and your children ( if they are at issue in the lawsuit)
  5. If Children are involved in the lawsuit: cost of daycare, camps, after care, extra-curricular activities and private school; a copy of any child support orders already in place.
